*Person with Disability **Those who are obtaining application form from Bank/IITR JAM office should send  the  Demand Draft, if opting for a second test paper.
Demand draft should be drawn in favour of “Chairman, JAM 2011, IIT Roorkee”, on any bank, payable at Roorkee. The completed OFFLINE/ONLINE Application Form with necessary enclosures must reach the “Organizing Chairman, JAM 2011, IIT Roorkee, Roorkee-247 667”, latest by 31st January 2011.
